Incident Report (Injury, Misconduct, Suspected Abuse, or Concerning Behaviour)

This form is intended to document any incident involving injury, misconduct, suspected abuse, or any behaviour that raises concern for the safety or well-being of an individual. Prompt and accurate reporting ensures appropriate follow-up, care, and accountability. This completed report should be submitted as soon as possible to the appropriate Ministry Supervisor/Director and/or the Abuse Response Team (ART), depending on the nature of the incident.
